About the Author
Giovanni Maciocia is one of the most highly-regarded and respected authors and lecturers in the West. He is Visiting Associate Professor at the Nanjing University of Traditional Chinese Medicine and has many years of experience in teaching and practice. Giovanni trained initially in the UK and then in China, learned Chinese to be able to study from original Chinese texts, and was one of the first practitioners and authors to introduce Chinese medicine in a clear and accessible way to a Western student audience.

Excellent TCM Source

This is an excellent book for TCM practitioners. It covers a wide variety of diagnostic techniques all included in one book (rather than having to buy 20 short books with similar info). From face diagnosis, to nail diagnosis, to in depth pattern recognition (for thin necks to swollen ears to stomach aches), Maciocia has included it all. I found the part about using palm lines to show tendencies towards certain types of diseases very interesting. There are colour plates in the back of the book that show examples of body type, tongues, and skin conditions.
The only problem I see is that it can be a little hard to find what you are looking for until you get used to using the book. With all of the info covered, Maciocia included a "How to use this book" section in the begining to help with that.

If you have the Foundations book, you have this book

It's a very large book with a very large price, but it's not worth the price. The entire section on the different Zang Fu patterns is virtually identical to The Foundations of Chinese Medicine: A Comprehensive Text for Acupuncturists and Herbalists. Second Edition.
I felt like I paid for a book that I already owned. There is definitely a plethora of other information in this book too, but it's presented in an overwhelming manner that is also fairly repetitive. Anyone who has seen the previous Foundation text (the white and black one) and then read the latest edition, knows what I mean about the same information being repeated over and over. Think of those blue call-out boxes and the hard to follow charts with arrows pointing everywhere. Unfortunately, this book follows in the same style. Ideally, it should have been half the size to double its appeal.

The truth of the matter.

I cannot agree with Lisa W. The chapter dedicated to diagnosis in Foundations of Chinese Medicine is about 95 pages long; the book Diagnosis in Chinese Medicine runs to about 1000 pages, so I do not see how they could be the same. Some overlap between Foundations and Diagnosis is inevitable (Zangfu patterns) but the sections are not the same. To me, this book is the most comprehensive source on Chinese diagnosis. Two aspects stand out for me. One is the comprehensive list of symptoms and signs with patterns causing them and cross-reference to other chapters; the other is the section on pulse diagnosis which, to me, is full of useful clinical insights. As for the arrows going in all directions...that is how Chinese medicine is; there are no rigid, one-way correspondences (e.g. Phlegm aggravates Blood stasis but this aggravates Phlegm).
